Hallux Rigidus Treatment: Big Toe Arthritis Diagnosis, X-Rays, and Conservative Care
Hallux rigidus is arthritis and stiffness of the big toe joint. It can make walking, exercising, and even wearing certain shoes uncomfortable. Many people notice pain when pushing off, going up stairs, or wearing shoes that bend at the front.

Common hallux rigidus symptoms
- Pain on the top of the big toe joint (especially with push-off)
- Stiffness and reduced big toe motion
- Swelling around the joint
- A bump on the top of the joint (bone spur)
- Pain with certain shoes, especially flexible or tight toe-box shoes
- Pain with running, lunges, or stairs
What causes hallux rigidus?
Common contributors include:
- Wear-and-tear arthritis over time
- Prior injury to the big toe joint (turf toe, jamming injuries)
- Foot mechanics that overload the joint
- Genetics (some people are more prone to joint arthritis)
Diagnosis: why X-rays matter
Hallux rigidus is diagnosed based on symptoms, exam, and X-rays.
X-rays are important because they show:
- Joint space narrowing (arthritis)
- Bone spurs (especially on the top of the joint)
- Alignment issues that may affect treatment choices
- Severity/stage of arthritis
Conservative treatments that work
1) Shoe changes (often the biggest improvement)
- Stiff-soled sneakers or shoes that don’t bend much at the toe
- A wide toe box to reduce pressure on the joint
- Avoid thin, flexible shoes that force the toe to bend
2) Inserts and support
- Prefabricated inserts or custom orthotics (when mechanics are contributing)
3) Activity modification
Temporarily reduce activities that require strong push-off (sprints, hills, lunges) while symptoms calm down, then return gradually.
4) Anti-inflammatory options
Depending on your medical history, treatment may include topical or oral anti-inflammatory options and targeted home care.
5) Cortisone injections (selected cases)
In the right patient, a cortisone injection can reduce inflammation and pain inside the joint and help you function more comfortably—especially when combined with supportive footwear.
6) Class IV FDA-cleared laser therapy
Laser therapy can be used as a conservative option to help reduce pain and inflammation and support tissue healing as part of a full plan—especially during flare-ups.
7) When conservative care isn’t enough
If pain and stiffness remain significant despite appropriate conservative care, more advanced options may be discussed after a full evaluation.
